Most complaints about a phone, internet, or TV provider come down to surprise fees, promised rates that changed, outages, and cancellations that keep billing. Here is the order that works: official support first, in writing, with records — then escalation.
Replace everything in brackets. Short and specific beats long and angry.
Subject: Formal complaint — [account or order number] To Northstar Mobile, On [date] I [what you bought or what service you used]. The problem is [what went wrong, in one or two sentences]. I contacted support on [date] and was told [what you were told]. Reference number: [number]. I am asking for [refund / replacement / correction / cancellation] by [date, about 10 business days out].
